Appointment booking
Appointments can be made by contacting our office during business hours on (03) 9067 7034. A valid referral from your GP or specialist will be required in order to claim the Medicare rebate for your consultation.
Appointment cancellation
Please advise us 24 hours prior to your appointment if you need to cancel or reschedule. Failure to do so may incur a cancellation fee.
What to bring to your consultation
Upon booking your appointment, our reception staff will provide you with a patient details and privacy policy form. Please fill these in and send back to our officer prior to your appointment. Please also ensure you bring along your referral letter and any previous x rays, scans, pathology or test results that may be related to your condition.
